When children enter adolescence, they often start making more decisions about their lives, including decisions related to their health. However, many adolescent patients have not learned how to manage their disease nor received the support they need to transition to adult care. This webinar will highlight peer-to-peer support as a resource for adolescents transitioning to adult care.
